Midwifery is coded separately from Registered Nursing under ANZSCO 254111. If you hold dual qualifications (RN + Midwife), your nominated occupation must reflect your primary skilled role — these are distinct registration categories with AHPRA and are assessed independently.

The skills assessment authority is ANMAC. Note that NMBA/AHPRA handles your midwifery registration — a separate step that follows a positive ANMAC outcome, not a substitute for it.

For midwives, the strongest advantage lies in regional demand. Rural and regional maternity units across Australia face genuine staffing gaps — securing a regional offer simultaneously satisfies 491's work obligation and dramatically boosts your points score. After two years of regional work, you can transition to 191 permanent residency.

Points composition — age, English, experience, qualifications — works the same as every skilled occupation. What differs for midwives is that Superior English (IELTS 8 overall) adds 20 points, a lever worth targeting given ANMAC's already-high English bar.

Employer Sponsorship for Midwives: A Realistic and Well-Worn Path

For Midwives, employer sponsorship via 482/Skills in Demand is one of the most accessible routes to Australia — obstetric units across public and private hospitals actively sponsor, and the TSMIT of AUD $73,150 is rarely a barrier given midwifery award rates.

Midwifery sits on the CSOL, making it eligible for the 482 Medium-term Stream (up to 4 years) and the 186 ENS pathway. Sponsors are not hard to find — experienced obstetric employers know the process well and move efficiently.

  • Public hospital maternity units — the most common and experienced sponsors
  • Private obstetric hospitals — particularly in metro areas
  • Regional and rural health services — structural midwifery shortages make these highly motivated sponsors
  • Community midwifery services — smaller but growing pipeline

Midwifery award wages consistently exceed the TSMIT of $73,150, so salary compliance is not a practical obstacle. Regional positions are especially valuable — a regional employer sponsor can simultaneously satisfy your 491 regional work obligation on the path to 191 PR, and after 2 years with the same employer you may apply for 186 ENS (Temporary Residence Transition stream).

ANMAC Skills Assessment for Midwives: Harder Than It Looks

The ANMAC assessment for Midwives is a standalone process entirely separate from AHPRA registration — and confusing the two is the single most common reason applications stall.

ANMAC evaluates whether your midwifery education and clinical obstetric practice are equivalent to the Australian standard. Proving adequate clinical hours — including documented delivery numbers — is the make-or-break hurdle for most internationally trained midwives.

The most common pitfalls VJCE sees with Midwife assessment files:

  • Conflating RN and Midwife assessments — your nursing qualification is assessed separately; only midwifery-specific education and practice counts here
  • Insufficient clinical hours evidence — ANMAC expects documented obstetric caseload (e.g. deliveries conducted); a general reference letter is not enough
  • Reference letters missing obstetric responsibility detail — referees must speak specifically to your autonomous midwifery practice, not just your ward duties
  • Expired English results submitted — IELTS scores must be valid at the time of assessment lodgement
  • Non-NAATI certified translations — any document not in English must be translated by a NAATI-accredited translator or it will be rejected outright

Note: ANMAC and AHPRA/NMBA are two separate bodies with two separate processes. A positive ANMAC skills assessment does not grant AHPRA registration — you must complete both steps before you can work as a midwife in Australia.

§ 5 Common Problems & Refusals

Common Failure Modes for Midwife Applicants

The single biggest reason Midwife applications fail is confusing midwifery assessment with registered nurse assessment — they are entirely separate processes with different evidence requirements.
  • Wrong assessment pathway: Submitting an RN-category ANMAC assessment instead of a midwifery-specific one (ANZSCO 254111) invalidates the entire application.
  • Insufficient obstetric clinical hours: ANMAC requires documented evidence of hands-on midwifery practice — delivery counts, antenatal/postnatal caseloads — not general nursing hours.
  • Weak reference letters: Referees must specify midwifery-specific responsibilities. Letters that describe generic ward duties are routinely rejected or trigger RFIs.
  • English scores lapsing: ANMAC requires IELTS 7 in all four bands. Scores older than two years at time of assessment submission are invalid — a critical and avoidable delay.
