What is OVHC insurance and is it mandatory for a 485 visa application in Australia?

Overseas Visitor Health Cover (OVHC) is mandatory for Subclass 485 visa applicants, and you must hold it for the full duration of the visa grant period as a visa condition (Condition 8501).

OVHC is a private health insurance product designed specifically for temporary visa holders who are no longer eligible for Overseas Student Health Cover (OSHC). OSHC covers you while you hold a student visa (Subclass 500), but once you transition to the Temporary Graduate Visa (Subclass 485), OSHC is no longer the appropriate product — you need OVHC instead.

Visa Condition 8501 is imposed on every Subclass 485 visa. It requires the visa holder to maintain adequate health insurance for themselves and any secondary applicants (such as a partner or dependent children) for the entire period the visa remains in effect. Failing to maintain this cover is a breach of visa conditions and can lead to visa cancellation.

Major OVHC providers accepted by the Department of Home Affairs (DOHA) include Medibank, Bupa, Allianz Care, nib, and Peoplecare. What health insurance (OVHC) is required for a 485 visa application?

You need an OVHC policy that provides hospital and ambulance cover at minimum, effective from your 485 visa grant date, covering all persons included in your application.

DOHA does not mandate a specific OVHC product tier, but the policy must meet the "adequate health insurance" standard referenced in the Migration Regulations. In practice, this means the policy must include:

What happens if there is a gap in health insurance coverage when applying for a 485 visa?

A gap in health insurance coverage between your OSHC expiry and your OVHC start date does not directly cause your 485 application to be refused, but it can raise compliance concerns and must be carefully managed.

The key distinction is between the pre-lodgement period and the post-grant period. DOHA does not require continuous health insurance coverage in the period before you lodge your 485 application. However, once your 485 visa is granted, Condition 8501 applies from day one of the visa grant, and any lapse after that point is a visa breach.

The practical risk of a pre-lodgement coverage gap is lower than many applicants fear, but it is not zero. A case officer reviewing your application may query why there was a gap if it is evident from your documents. More importantly, a gap means you are uninsured during that period — in Australia's private healthcare market, even a short hospitalisation without insurance can result in significant out-of-pocket costs.

Our standard advice at VJ Consulting and Education is to arrange your OVHC policy so that it commences the day after your OSHC expires, eliminating any gap entirely. Most providers allow you to set a future commencement date when purchasing a new OVHC policy.

Does OVHC need to be active before or on the date of 485 visa application?

Your OVHC does not need to be active on the date you lodge your 485 application, but it must be in place and active from the date your visa is granted.

This is one of the most commonly misunderstood timing questions we see. When you lodge your 485 application, DOHA requires evidence that you have arranged adequate health insurance, but the policy does not need to have already commenced. Many providers allow you to purchase a policy with a future start date — for example, you can purchase OVHC today and set it to begin in two months, which is perfectly acceptable.

What you must include with your application is documentary evidence that health insurance will be in place from the date of grant. This is typically a policy certificate or a letter from your insurer confirming the policy, the commencement date, and the coverage period.

Note: If your visa is granted earlier than expected — which does happen — ensure your OVHC policy is set to commence no later than the expected visa grant date. Some applicants set their OVHC to start on their anticipated grant date and then find the visa is granted before that date, leaving a short gap. To be safe, set the OVHC commencement date to coincide with the lodgement date itself.

Can I submit a 485 visa application before completing my health examination?

Yes — you can lodge your 485 application before completing your health examination, and DOHA will typically send you a health examination request after lodgement.

Under the current DOHA process for the Subclass 485 visa, health examinations (also called medical examinations) are not always required upfront. The standard approach is:

  1. Lodge your 485 application with all available documents, including your OVHC evidence.
  2. DOHA assesses the application and, if a health examination is required, issues a request through your ImmiAccount.
  3. You complete the health examination at a DOHA-approved panel physician within the timeframe specified in the request.
  4. Results are transmitted electronically to DOHA by the panel physician — you do not need to submit them manually.

However, some applicants choose to complete a health examination before lodging, particularly if they are aware that health-related conditions may need to be disclosed or if they want to proactively demonstrate compliance. This is a legitimate strategy but is not required by DOHA.

Importantly, if you have already completed a health examination within the last 12 months for another visa application, DOHA may waive the requirement to repeat it for the 485 application, depending on the results and your circumstances.

Does Medibank allow direct transfer from OSHC to OVHC for the 485 graduate visa?

Yes — Medibank does allow a direct transfer from an existing OSHC policy to an OVHC policy, and this is one of the most administratively convenient options for 485 applicants.

Medibank is one of the largest health insurers serving international students and graduates in Australia, and the OSHC-to-OVHC transition process is a well-established product pathway. When transferring, Medibank can typically:

The key advantage of a direct transfer is that it minimises or eliminates any gap in coverage and simplifies the documentation you need to include with your 485 application — you can provide a single document showing continuous cover from your OSHC period through to your OVHC period.

Does everyone need a health check for a 485 Temporary Graduate Visa application?

Not everyone is required to complete a health examination for the 485 visa — whether one is required depends on your nationality, country of residence, and individual health history.

DOHA uses a risk-based framework to determine whether a health examination is required for a particular applicant. The key factors include:

  • Country of birth and recent country of residence: Applicants from certain countries are required to complete a chest X-ray (for tuberculosis screening) regardless of other factors. This includes many countries in South and Southeast Asia, sub-Saharan Africa, and parts of the Pacific.
  • Length of stay: For visa applications that would allow a stay of more than six months, health examinations are more commonly required.
  • Previous health declarations: If you disclosed a health condition in a previous visa application, DOHA may require an updated examination.
  • Age: Children under a certain age may be exempt from some components of the health examination.

For the majority of 485 applicants who are Indian, Chinese, Pakistani, Nepalese, or from other high-health-check-requirement countries, a full health examination including chest X-ray will be required. Citizens of countries such as the UK, USA, Canada, New Zealand, and most Western European nations are less likely to face a mandatory health examination, though DOHA retains the discretion to request one.

Note: Secondary applicants (partners and children) included on your 485 application are subject to the same health examination requirements as the primary applicant. Do not overlook dependants when planning your health check timeline.

Can I add health insurance after submitting my 485 visa application?

Yes — you can provide updated OVHC evidence after lodgement if it was not included with your initial application, but this should be done as promptly as possible.

If you lodged your 485 application without including OVHC documentation — perhaps because your policy was not yet arranged — you can upload the evidence through your ImmiAccount at any time before a decision is made. DOHA case officers generally allow applicants to provide outstanding documents, and health insurance evidence is one of the most commonly submitted post-lodgement documents.

However, there are risks to leaving this until after lodgement:

  • If DOHA issues a decision before you upload your OVHC evidence, the application may be refused on health insurance grounds.
  • Processing times have been unpredictable. Some 485 applications are decided very quickly — within days in some cases — so assuming you will have time to add documents after lodgement is not a safe strategy.
  • A DOHA case officer may issue a Notice of Intention to Refuse (NOIR) if they cannot see health insurance evidence, giving you a short window to respond — typically 28 days.

Our strong recommendation at VJCE is to have your OVHC policy arranged and your certificate uploaded at the time of lodgement. This removes one of the most avoidable grounds for delay or refusal.

Is an IELTS score of 6.5 with one band below 5.5 acceptable for the 485 visa?

No — an IELTS score of 6.5 overall with any single band below 5.5 does not meet the English language requirement for the Subclass 485 visa under most pathways.

The English language requirement for the Subclass 485 visa (Graduate Work stream and Post-Study Work stream) is one of the following:

An IELTS score of 6.5 overall is above the minimum overall threshold of 6.0 — but the per-band minimum is 5.0, not 5.5. Therefore, a score of 6.5 overall with one band at 5.0 would be acceptable. However, a band below 5.0 would fail the requirement regardless of the overall score.

It is worth noting that if you are also applying for a points-tested visa (such as Subclass 189 or 190) and want to claim English language points above the minimum, you will need to meet higher band thresholds — typically 7.0 or 8.0 per band for Superior or Proficient English. The 485 visa itself only requires the minimum thresholds above.

Do you need to complete medical examination before or after lodging a 485 visa extension?

For a 485 visa extension (where available), DOHA follows the same process as initial applications — you lodge first, and DOHA requests the health examination after lodgement if one is required.

The term "485 visa extension" requires some clarification. The Subclass 485 visa is typically a one-time grant — there is no standard "extension" pathway in the same way as some other temporary visas. However, applicants who were granted a shorter period (for example, under the Graduate Work stream) may in some cases apply for a subsequent grant under the Post-Study Work stream if they have completed additional study, or they may apply for a different visa altogether.

For those re-applying for a subsequent Subclass 485 visa where eligible, the health examination requirements are assessed afresh. If DOHA already has a recent health examination on file (generally within the past 12 months) and your results were satisfactory, they may not require you to repeat the examination. However, you should not assume this — DOHA retains full discretion to request a new examination.

The practical steps for a re-application or subsequent 485 application are:

  1. Lodge the new 485 application before your current visa expires to maintain bridging visa protection.
  2. Ensure your OVHC is current and covers the new application period.
  3. If DOHA requests a health examination, complete it at a panel physician promptly — delays in completing the examination will delay the decision on your visa.
  4. Do not attend a health examination at a non-approved panel physician — results from unapproved providers are not accepted by DOHA.
Note: Panel physician appointments can sometimes have waiting periods of one to two weeks, especially in peak periods around the end of academic semesters. If you receive a health examination request, book your appointment immediately rather than waiting.
